- Downloadable service request - Download and fill out one of these forms, then submit via fax (355-5205) or campus mail (to Infrastructure Planning and Facilities):
Note: Projects estimated to cost more than $20,000 cannot use the online service request. Those projects must be submitted using the downloadable (PDF or Excel) forms, following the Manual of Business Procedures policy. To ensure integration with existing University facilities and to comply with safety and building codes, statutes, regulations and University specifications, all alterations and improvements to MSU facilities must receive proper authorization. See Section 201 of the Manual of Business Procedures.
